The higher the lesion, the less obvious the deformity.
সাধারণত ইনজুরি যত উপরে হয়, ক্ষতি তত বেশি হওয়ার কথা। কিন্তু ulnar nerve-এর ক্ষেত্রে elbow-তে (high lesion) ইনজুরি হলে হাতের deformity কম হয়, আর wrist-এ (low lesion) ইনজুরি হলে deformity অনেক বেশি severe হয়। এটাই প্যারাডক্স।
1. Ulnar Lesion at the Elbow (High Lesion)
❇️Mechanism: Elbow লেভেলে ইনজুরি হলে হাতের intrinsic muscles-এর সাথে Flexor Digitorum Profundus (FDP)পেশীটিও paralyzed হয়ে যায়।
Result: FDP অবশ থাকায় ring এবং little finger-এর DIP joint-এ কোনো flexion হতে পারে না।
Deformity: Mild Claw Hand (কম স্পষ্ট Deformity).
2. Ulnar Lesion at the Wrist (Low Lesion)
❇️Mechanism: Wrist লেভেলে ইনজুরি হলে intrinsic muscles paralyzed হয়, কিন্তু FDP muscle intact (সচল) থাকে, কারণ এটি elbow-র নিচেই নার্ভ সাপ্লাই পেয়ে যায়।
Result: সচল FDP পেশীটি fingers-এর DIP joint-কে শক্তিশালীভাবে flex (ভেতরের দিকে টেনে বাঁকা) করে ফেলে।
Deformity: Severe Claw Hand (খুব স্পষ্ট Deformity)
