DTM définition : au secours la confusion revient !!!
Je ne parle pas de 1980, mais de 2023 !

Le DTM ce n’est que l’ATM !!
Un article récent d’une équipe du London Hospital fait ressortir du placard les discussions sur le terme DTM. Faisant référence à une classification des douleurs oro-faciales proposée en 2000 par l’international Headache Society (IHS) , Ces auteurs définissent le DTM comme « une douleur localisée de l’ATM….. » (limitée à l’ATM) alors que ce n’est pas ce qui est écrit dans la classification citée en référence. Et ceci dans une r***e prestigieuse les « OOO » !!

Le DTM n’existe plus !!
En fait, la nouvelle « classification des douleurs oro-faciales » de l’IHS est elle aussi très confuse, ne voulant pas reconnaître le terme de DTM (tout en l’utilisant…) pour le séparer en deux catégories « Myofascial orofacial pain » et « TemporoMandibular Joint Pain »
On comprend que ce club de « douloureux » ne s’intéresse qu’à la douleur, mais il existe aussi des troubles dysfonctionnels de l’ATM non douloureux. On sait aussi qu’il existe des troubles musculaires d’origine articulaire. Ces deux faits légitiment parfaitement le terme global de myoarthropathie de l’appareil manducateur si magnifiquement définit par Sandro Palla.
Probablement, il y a des choses que je n’ai pas bien comprises, aussi Il serait intéressant d’avoir quelques explications de la part nos amis qui ont fait partie du groupe de travail de l’IHS.

Mon opinion est que le terme DTM est mauvais (c’est dit depuis 30 ans….) mais il est consacré par l’usage, donc on le garde. ASSEZ de confusion.
Il se manifeste des formes articulaires, musculaires et musculo-articulaires, comme il comprend des formes primaires (idiopathiques = dysfonctionnelles) et des formes secondaires liées à des causes régionales ou générales (systémiques)

DTM is limited to TMJ !!
A recent article from a team at the London Hospital brings pushed out of the closet. discussions about TMD term. Referring to a classification of orofacial pain proposed in 2000 by the International Headache Society (IHS), these authors define TMD as “pain localized to the temporomandibular joint (TMJ)” (limited to the TMJ) while this is not what is written in the referenced classification. And this in a prestigious magazine “OOO”!!

TMD no longer exists !!
In fact, the new IHS “classification of orofacial pain” is also very confusing, no recognizing the term TMD (while using it…??) to separate it into two categories “Myofascial orofacial pain” and “TemporoMandibular Joint Pain”
We understand that this “painful” club is only interested in pain, but there are also non-painful dysfunctional TMJ disorders. We also know that there are muscular disorders with joint origin. These two facts perfectly legitimize the global term of myoarthropathy of the masticatory system magnificently defined by Sandro Pala.
Probably, there are things that I didn't quite understand, so it would be interesting to have some explanations from our friends who were part of the IHS working group.

My opinion is that the term TMD is poor (it has been said for 30 years….) but it is established by usage, so we keep it. ENOUGH of the confusion.
TMD manifests itself in articular, muscular and musculo-articular forms, as it includes primary forms (idiopathic = dysfunctional) and secondary forms linked to regional or general causes (systemic).
