🔹 Quick Tip: While the hard copy stays with the office for now, you are highly encouraged to take a clear photo of it for your personal records and immediate reference! 📸
🔹 Form 137-A Request: Use the provided Request Slip to secure your official transcript from your previous school as soon as possible.

Your enrollment confirmation will be processed once your complete original documents are submitted, and your student numbers will be forwarded to our department head within 𝟱–𝟳 𝘄𝗼𝗿𝗸𝗶𝗻𝗴 𝗱𝗮𝘆𝘀.

STEP 1: Document Submission

Submit all required documents to the department head/assigned personnel. Please bring the original copies for the requirement along with their respective photocopies.

📌Note: If you already submitted some of your documents, you just need to submit the remaining requirements.

STEP 2: Freshmen Enlistment

Proceed to the faculty room for your official enlistment into the program/course.
