Here are a few important reminders regarding your documents:
๐น Original COR: Your original Certificate of Registration will be officially distributed on the first day of classes.
๐น Quick Tip: While the hard copy stays with the office for now, you are highly encouraged to take a clear photo of it for your personal records and immediate reference! ๐ธ
๐น Form 137-A Request: Use the provided Request Slip to secure your official transcript from your previous school as soon as possible.

Submit all required documents to the department head/assigned personnel. Please bring the original copies for the requirement along with their respective photocopies.
