Expansion Matters Post-Thaw Expanded Blastocyst:

Post-thaw re-expansion is a vital indicator of embryo viability. Expanded blastocysts (which re-hydrate and resume active growth quickly) boast significantly higher implantation and live birth rates. Non-expanded or completely shrunken blastocysts show markedly lower success rates, though they still retain pregnancy potential .

Why Expansion Matters Post-Thaw Expanded Blastocyst:
The embryo successfully absorbs fluid into its central cavity (the blastocoel), pressing against the outer shell (zona pellucida) to prepare for hatching. Fast, robust re-expansion signals high cellular health and survival .

The Clinical Assessment Process :
Normal Initial State: When a blastocyst is thawed, it often appears completely shrunken or collapsed. This is a normal part of the rapid-freezing (vitrification) process, which safely dehydrates the embryo to prevent ice crystal damage.Waiting Period: Embryologists typically culture the embryo for 1 to 4 hours post-warming to evaluate its ability to recover and rehydrate.
Evaluation Criteria: Research shows that embryos that re-expand to at least 90% of their baseline size have the strongest statistical chance of clinical pregnancy .

*Key Steps in the Post-Thaw Transfer Protocol :

- Extended Post-Warm Culture: Embryologists often culture the thawed blastocyst for 2 to 4 hours (and sometimes up to 20 to 22 hours). This prolonged incubation allows biological activity to restart and gives theR blastocoel cavity time to re-expand.

- Assisted Hatching: Often performed on frozen-thawed blastocysts—especially those with a thicker zona pellucida or those that remain shrunken—to assist the embryo in escaping its outer shell after transfer.

- e-Evaluation: Embryologists assess viability and re-expansion before the actual transfer. While quick re-expansion is a positive marker, data shows that even embryos that show slow or no expansion prior to transfer can result in successful pregnancies.

- Endometrial Preparation: A programmed or natural FET cycle is carefully timed to ensure the uterine lining is at the optimal thickness and receptivity window (usually corresponding to ≥ 7-8 mm with a trilaminar appearance) prior to the transfer procedure.

There is only one randomized controlled trial (RCT), and it failed to demonstrate that AI-based selection is as effective as conventional selection methods (meaning AI performed no better than traditional morphological assessment for sure ).

Intrauterine hCG before embryo transfer as an add-on treatment to improve implantation. This involves infusing hCG directly into the uterus shortly before transfer.

In this individual participant data meta-analysis, Zou et al. (2026) combined data from 7 randomized trials involving 2,244 IVF patients. They found no improvement in live birth, clinical pregnancy, miscarriage, or ectopic pregnancy rates.

Researchers reviewed the original patient data and screened studies for research quality and trustworthiness before including them. Only 7 of 28 studies were included .

These studies generally found no benefit from intrauterine hCG before embryo transfers and should not currently be offered as a routine IVF add-on.

PCOS Renamed PMOS in Landmark Global Consensus

Why PCOS Was Renamed to PMOS

The name "PCOS" was deemed misleading and inaccurate, focusing too narrowly on "cysts" and ovaries, which delayed diagnosis and marginalized the condition's severe systemic health impacts.

Misleading Terminology: Many individuals diagnosed with the condition do not actually have abnormal ovarian cysts. The "polycystic" appearance is often just an accumulation of underdeveloped follicles.

Systemic Nature: The condition is a complex endocrine (hormonal) and metabolic disorder affecting heart health, insulin, weight, and mental health, not just a gynecological issue.

Delayed Diagnosis & Stigma: The focus on infertility and ovaries led to missed diagnoses and a lack of holistic treatment, often dismissed by doctors.

Research in a 2026 study found that bacteria detected on the embryo transfer catheter were linked to lower live birth rates, likely reflecting the patient’s own microbiome being carried into the uterus during transfer.
During embryo transfer, the catheter passes through the va**na and cervix, which are not sterile, meaning bacteria can be carried into the uterus and this may affect implantation.
A study by Boughanmi et al. (2026) measured bacteria in the va**na, cervix, and on the catheter after transfer, and compared this to pregnancy outcomes.
They found that when certain bacteria were present, live birth rates were much lower, while patients with more Lactobacillus had better outcomes. These bacteria were more common in patients with bacterial vaginosis (BV), a common and often unrecognized imbalance of the va**nal microbiome.
This suggests that the embryo transfer procedure itself may carry bacteria into the uterus, and that differences in the va**nal microbiome could quietly affect success rates.
The authors highlight BV as important, noting it can be screened for with a simple test. However, the study didn’t test whether treating BV or changing the microbiome improves outcomes, so this still needs to be confirmed.
