📌 Requirements:

1. Original and photocopy of Birth Certificate
2. Original SF9 (Report Card)
3. Proof of Residency (any bill reflecting the address and name of parent/legal guardian)
4. Affidavit of Legal Guardianship (if not living with parent)
5. Photocopy of Certificate of Completion (for Grade 10 completers) / ALS Certification (for ALS passers)
6. Updated diagnosis/medical record (for SNED learners)

✅ Please strictly follow the assigned enrollment schedule to ensure an orderly and efficient process.
✅ The number of enrollees will be accommodated based on the school’s absorptive capacity.
