REQUEST FOR ACCOMPLISHMENT OF GRADUATE TRACER SURVEY FORM
(For Cohorts 2019 and 2021 Awardees)

In line with the Commission on Higher Education’s (CHED) commitment under the ACHIEVE Agenda to promote graduate employability, responsive higher education programs, and evidence-based policy development, all graduates who were beneficiaries of CHED Scholarship and Student Financial Assistance Programs under Award Years 2019 and 2021 are requested to accomplish the Graduate Tracer Survey Form.

CHED SCHOLAR GRADUATES TRACER This form is used to monitor and record the graduation details of CHED scholars. The information collected will help track the completion of scholars’ academic programs and support CHED in evaluating the outcomes and impact of its scholarship programs.

The City Government of Masbate, under the leadership of Mayor Atty. Ara T. Kho and with the special endorsement of 2nd District Representative Congresswoman Dra. Olga T. Kho, is pleased to announce that applications for the CHED Dunong Tulong Program are now officially open.

This program is dedicated to supporting qualified students in achieving their academic goals.

𝗪𝗛𝗢 𝗖𝗔𝗡 𝗔𝗣𝗣𝗟𝗬? (𝗤𝗨𝗔𝗟𝗜𝗙𝗜𝗖𝗔𝗧𝗜𝗢𝗡𝗦)

To qualify for this financial assistance, applicants must meet the following criteria:

✅ Must be an incoming 4th Year College Student who was actively enrolled during the First Semester of School Year 2025–2026 as a 3rd Year student
✅ Must be a bonafide resident of Masbate City
✅ Must be currently studying within Masbate City
✅ Must NOT be an Educational Aid Grantee of the City Government of Masbate
✅ Must NOT be a UNIFAST Grantee, as this assistance program is also under the UNIFAST Program

Only 290 slots are available and will be accommodated on a FIRST-COME, FIRST-SERVED basis.

In line with CHED’s data gathering regarding the proposed enhancement of the Tertiary Education Subsidy (TES) under RA 10931, all TES Scholars are requested to accomplish the Student Expenses Survey on or before May 29, 2026, 12:00 midnight.

4. LandBank CRF (PLEASE SECURE THIS AT THE SCHOLARSHIP SECTION OFFICE.)
