𝟐. Print all the downloaded forms on A4-sized paper and fill them out completely.
𝟑. Take the Laboratory Request form to any public or private hospital or clinic to have the required laboratory tests performed.
𝟒. After completing the tests, bring the results with you to your enrollment appointment and present them to the DEBESMSCAT Medical Officer to receive your medical certificate.
𝟓. Bring all completed forms: Individual Student Inventory (ISI) form, Student’s Health Record, and Data Consent form on your enrollment date.

Claim your individual Result Slip, and submit your accomplished Individual Student Inventory Form.
𝗦𝘁𝗲𝗽 𝟮: COLLEGES
Proceed to the college where you have chosen your course.
- For the College of Engineering qualifiers, you must take a short qualifying test. (Please bring a scientific calculator.)
- For the College of Education and the BS Agriculture program qualifiers, you must attend a face-to-face pre-screening interview.
If you successfully pass and meet all admission criteria in your respective assessment, you will be offered a slot in the program and then issued a certificate of passing.
- For qualifiers in non-board programs, No Interview will be conducted; instead, you must request a certificate of acceptance from the department heads or the college of your chosen course before you enroll.
𝗦𝘁𝗲𝗽 𝟯: MEDICAL SERVICES
Submit your completed Students’ Health Record Form, your original chest x-ray (PA View), and drug test results. If you are applying for the BSIT Food Technology program, you must also submit an additional test result for Hepatitis B. The College Medical Officer will issue a Medical Certificate upon receipt of all required laboratory test results.

Submit all required enrolment documents for evaluation and verification. If everything is complete, the assigned staff will process your enrollment and provide you with an Enrollment and Assessment Form, which will be printed and stamped with “Officially Enrolled.” If you are not covered by RA 10931, proceed to the Cashier’s Office to pay any fees, then return to the Office of the Registrar for validation and to have the Enrollment and Assessment Form stamped with “Officially Enrolled.”

✅ Must be an incoming 4th Year College Student who was actively enrolled during the First Semester of School Year 2025–2026 as a 3rd Year student
✅ Must be a bonafide resident of Masbate City
✅ Must be currently studying within Masbate City
✅ Must NOT be an Educational Aid Grantee of the City Government of Masbate
✅ Must NOT be a UNIFAST Grantee, as this assistance program is also under the UNIFAST Program

To qualify for this financial assistance, applicants must meet the following criteria:

✅ Must be an incoming 4th Year College Student who was actively enrolled during the First Semester of School Year 2025–2026 as a 3rd Year student
✅ Must be a bonafide resident of Masbate City
✅ Must be currently studying within Masbate City
✅ Must NOT be an Educational Aid Grantee of the City Government of Masbate
✅ Must NOT be a UNIFAST Grantee, as this assistance program is also under the UNIFAST Program

Only 290 slots are available and will be accommodated on a FIRST-COME, FIRST-SERVED basis.

• Certificate of Indigency (issued by your Barangay)
• Certificate of Enrollment for the 1st Semester of SY 2025–2026
• Photocopy of Valid School ID (bring the original for verification)
• Two (2) pieces 2x2 ID Picture
• Duly Accomplished Application Form
