📂 General Requirements for Application
Applicants must secure and submit the following documents to the Laboratory Schools Principal's Office during the application period:
1. Photocopy of Report Card (Form 138/SF9) with at least 90% GWA
2. Certificate of Good Moral Character (Original)
3. Photocopy of PSA Birth Certificate
4. 4pcs recent 2x2 ID Picture
5. Testing Fee (P200.00)
