Fortunately, most cases are preventable and can be treated in their early stages.

📢 Ongoing Health Examination and Issuance of Medical Certificate for incoming first year students.

Note: There is no announced deadline for submitting laboratory results for Medical Exam. Medical examinations are accommodated throughout the year for incoming students and currently enrolled students who require clearance for various purposes.

| ISSUANCE OF MEDICAL CERTIFICATES STARTING JUNE 1, 2026

The CatSU Infirmary / Health Services will begin accepting applications for medical certificates starting June 1, 2026 for Incoming First-Year Students, Transferees, and Graduate School Applicants, Academic Year 2026–2027.

CLINIC SCHEDULE:
Monday to Thursday | 8:00 AM – 5:00 PM

REQUIREMENTS:
• Accomplished Medical History Form

• Copy of the following laboratory results:
✅Chest X-ray
✅Complete Blood Count (CBC)
✅Drug Test
(Laboratory request form is available at the infirmary)

PROCEDURE:
1. Fill out the Medical History Form.
2. Secure a priority number from the clinic personnel.
3. Undergo vital signs assessment (height, weight, temperature, pulse rate, and blood pressure).
4. Proceed to health screening/assessment by the medical team.
5. Claim your Medical Certificate signed by the Medical Officer.

NOTE:
Incomplete requirements will NOT be processed.
No Medical History Form + No Laboratory Results = No Medical Certificate

All applicants must complete the requirements and procedures to be issued a “Fit to Enroll” certification.
