The success of the previous edition of Baltic Hernia Days in 2024 did not make us complacent. On the contrary — it became an obligation. Over the past two years, many congresses and conferences have taken place, and some of the ideas that once seemed new, fresh, and bold were later adopted and repeated by other events. We see this as a natural part of the field’s evolution, but at the same time, we know that the same patterns cannot be reproduced indefinitely.

That is why the new edition of Baltic Hernia Days will be more than a continuation. It will be a new beginning.

We are working on a programme that, both scientifically and organizationally, aims not merely to follow direction, but to set it.

Water. Fire. Earth. Air. Time. The elements that shape our biological existence and define our presence on this planet.

The previous edition guided us through operative techniques, surgical approaches, materials, and instrumentation. We began with the simplest tools and ended on the threshold of robotic surgery. We discussed meshes, indications, and the complex clinical pathways our patients may face.

We know perfectly well that hernia surgery has many dimensions, but its true progress is measured not in years, but in decades. Genuine breakthroughs are rare and usually emerge only from a handful of pioneering centers. That is why we fell silent for two years. And yet even today, surgery has not advanced so dramatically that an entire new congress programme could be built solely around new techniques.

So this time, we are evolving in a different direction.

We are returning to the roots of medicine — to the place where understanding is the foundation of therapy. Where etiology matters more than symptoms. Where distant relationships come together into one coherent whole.

To understand, not merely to believe.

This year’s motto is an invitation to look beyond the “what” in surgery and reach for the “why.” Because once we understand the “why,” the “what” becomes simpler. We will explore the origins of phenomena, the physical nature of tissues, and the hidden properties of synthetic materials. We will try to reveal the logic behind what too often is accepted merely as practice.

Perhaps for the last time.

The coming era of artificial intelligence may once again force us into belief — belief in processes whose mechanisms may lie beyond the full reach of human understanding.

And so, we invite you to Gdańsk for something more than a conference. We invite you to encounter an idea. To join a conversation about surgery that does not stop at technique. To attend the last supper of understanding.
