There is a common misconception that nurses only monitor vital signs and wait for others to make decisions. In reality, Nursing goes far beyond that.

During my years of working in the ward so far, I learnt that nurses are often the first to notice subtle changes in a patient's condition because we are consistently at the bedside. We assess, observe, interpret trends, and respond promptly when something is not right.

As nurses we also have our own nursing diagnoses, we focus on patient responses, risk, comfort, mobility, nutrition, coping, and many other aspects of care that directly influence outcomes .

We do not just record numbers on a chart. We connect those numbers with clinical signs, symptoms, and the patient's overall condition.

Many times, early deterioration is first identified by a nurse who is paying close attention. That is the value of Nursing continuous assessment, critical thinking, advocacy, and timely action.
Vital signs are only part of the story .

Nurses help read whole story
