Enroll your child !

Here are the 9 required documents to enroll :
1 .birth certificate
2. Physical Examination form (must be signed and dated)

3.current immunization records

4. Dental exam

5.proof of address

6.Parent/Gaardian Valid Drivers license/ state ID card

7. Guardianship documents (if applicable)

8. Medical insurance card for the child ( front and back )

9. Proof of house hold income

WE MUST HAVE ALL DOCUMENTATION TO COMPLETE THE ENROLLMENT PROCESS

Your child must be 3 to 5 years old to be enrolled into the CKA preschool program
